As part of our Center for Service Coordination department, you will be a key stakeholder in supporting our members. Your primary role will be outreaching to our members post-discharge to ensure all of their healthcare needs are met. Identification of the appropriate programs and services available is critical to the success of our members.



You'll enjoy the flexibility to work remotely from anywhere within the U.S. as you take on some tough challenges.


Primary Responsibilities:


  • Outreach calls to our members / beneficiaries to engage in clinical programs
  • Support the enrolled members / beneficiaries of our community partners
  • Collaborating with our PCP Network, physician offices and hospital facilities within Optum Care Network - Utah
  • Support members with coordinating healthcare services
  • Make appropriate referrals to the Customer Care team to support benefit, billing / payment and eligibility of our members
  • Support end to end referral process for all clinical programs
  • Notification to internal / external stakeholders on referral outcome
  • Assist with referrals / calls received by the Care Coordination Team, and complete community outreach and documentation

    Required Qualifications:


    • HS Diploma or GED
    • 1+ years of customer service experience analyzing and solving customer problems with great communications skills
    • Experience working within the healthcare industry
    • Professional experience in a clerical or administrative support related role
    • Working knowledge of medical terminology to communicate with members / beneficiaries and providers




      Preferred Qualifications:


      • Experience working with Centers for Medicare and Medicaid Services, especially Medicare and Medicare Advantage programs
      • Knowledge of community resources
      • Some college / undergraduate education
      • Medical Assistant (MA) certification

        California, Colorado, Connecticut, Nevada, New York, Rhode Island, or Washington Residents Only: The hourly range for California, Colorado, Connecticut, Nevada, New York, Rhode Island or Washington residents is $16.00 to $31.44.
