responsibilities:

o Performs patient assessments, coordination of care.
o Will be working with patients in a comfortable environment where the service is one-on-one.
o Performs other personal care services as necessary to meet the patient s needs.
Requirements: o Current State License as a Certified Nursing Assistant.
o Current CPR, First Aid, TB test, and physical.
We are offering a $1,000 signing bonus, contingent on agreement on a permanent assignment and 90 days of probation!

o Executes physician prescribed plans of care and compliant documentation of care provided in system of record.
o Performs various activities associated with daily living, communications, and monitoring of vitals while working at the patient s home.
o Communicates with family members, doctors, and other prescribed individuals regarding the health of patients.
o Prefer one year of experience as a Certified Nursing Assistant in a clinical care setting, home health preferred.
o Good interpersonal skills. o Prolonged standing and walking required, with ability to lift up to 50 lbs.

o Requires hand-eye coordination and manual dexterity.
