Job SummaryCurana Health is seeking a skilled Nurse Practitioner to join our team of healthcare professionals dedicated to providing high-quality care to seniors in skilled nursing facilities, assisted living communities, and memory care units. As a Geriatric Care Specialist, you will be responsible for managing clinical care for geriatric patients, building relationships with facility staff, and engaging with the multi-disciplinary care team to address quality of care and patient needs.Responsibilities

• Provide medical coverage for geriatric patients in partner facilities, including skilled nursing, long-term care, assisted living, independent living, and memory care facilities.
• Travel to facilities to provide medical coverage 4 days a week.
• Build relationships with facility staff, administrators, and nursing directors.
• Engage with the multi-disciplinary care team to address quality of care and patient needs.
• Complete timely documentation and billing for services provided per Curana policy.
• Respond to daytime calls from the facility when not physically present.
• Assist with the transition to the permanent Curana provider(s).
• Serve as a model and representative of Curana Health at all times.Benefits

    • NP degree.
    • Unrestricted CA license.
    • Unrestricted licensure in multiple states with the ability to obtain additional licenses within the first 30 days.
