Pay: Open to Negotiate POSITION SUMMARY We are seeking a
compassionate and skilled Home Health Occupational Therapist to
join our team. This role involves providing therapeutic services to
patients in their homes to help them regain independence and
improve their quality of life. The ideal candidate will have strong
clinical skills, a patient-centered approach, and the ability to
work independently. QUALIFICATIONS Accredited occupational therapy
degree. Current registration or license as required by state(s) of
practice. CPR certification. Strong verbal and written
communication skills. Licensed driver with insured automobile and
safe driving record. ESSENTIAL FUNCTIONS Perform assessments,
diagnostic tests, treatments, and evaluations under a medically
approved Plan of Care. Develop treatment programs and establish
goals for improved function. Document findings, plans,
interventions, and client progress. Communicate plans and changes
to the physician and Agency staff. Guide and instruct clients in
therapeutic self-care and activities. Advise and teach treatment
procedures to clients, family members, and Agency staff. Design and
order adaptive/assistive devices. Comply with Agency policies and
professional standards. Attend care conferences and participate in
care coordination. SUPERVISION OF OCCUPATIONAL THERAPY ASSISTANTS
Instruct and supervise Occupational Therapy Assistants. Available
by telephone to assist Occupational Therapy Assistants. Supervise
on-site every four to six visits, minimally once a month. About
