Job Summary: As a Nurse Practitioner or Physician Assistant in our Primary Care setting, you’ll provide hands-on, full-spectrum medical care to a diverse patient population. From managing chronic diseases to diagnosing acute illnesses, you’ll play a pivotal role in keeping your community healthy. This role offers a meaningful mix of autonomy and team collaboration in a well-established outpatient setting. Job Duties & Responsibilities: Conduct comprehensive physical exams and interpret lab and diagnostic results Diagnose and treat acute conditions and minor injuries Manage chronic diseases such as diabetes, hypertension, obesity, and heart disease Create evidence-based treatment plans tailored to patient needs and histories Deliver compassionate, thorough care that addresses physical and emotional well-being Collaborate with physicians, specialists, and nursing staff to coordinate care Educate patients and families about diagnoses, medications, side effects, and lifestyle changes Refer patients to specialists or coordinate hospital care as necessary Document all encounters and procedures in compliance with clinical and regulatory standards Conduct preventive screenings and engage in wellness promotion activities Stay current with continuing education and clinical best practices Maintain strict patient confidentiality and uphold all HIPAA regulations Prerequisites / License & Certification Requirements: Active Nurse Practitioner or Physician Assistant license in Arizona Graduate of an accredited NP or PA program DEA certification preferred No history of malpractice; must meet eligibility for payer credentialing If you meet all of our criteria and would like to be considered, please apply with your most updated Resume/CV.
