Position Summary: The Registered Nurse as a transition Nurse performs the functions of assessing, planning, implementing and evaluating care for patients.  Meets Standards of Care and Standards of Practice.  Supervises assigned personnel.  Promotes collaboration with physicians and personnel of other departments.  Demonstrates knowledge and skills necessary to provide care appropriate to patients served.  Demonstrates knowledge of principles of growth and development over life span and possesses ability to assess data reflective of patient status and interpret appropriate information needed to identify patient requirements relative to his/her age specific needs.  Provides care needed as described in department policies and procedures.  Completes a competency skills checklist within 3 months of employment.

 

KEY ACCOUNTABILITIES (includes, but is not limited to):

  • Demonstrates/verbalizes knowledge of position description/evaluations and scope of practice as defined in State of Nevada Nurse Practice Act and hospital and nursing service policies, protocols, and procedures.
  • Reviews patient assessment data including psychological, physical, environmental, self-care, education and discharge planning needs.
  • Reviews assessment/evaluation of patient/care partner(s) other ability to manage continuing health needs after discharge.
  • Teaches patient/significant other as appropriate to patient health care needs throughout the patient’s stay.
  • Demonstrates/verbalizes knowledge of principles of growth and development over life span and possesses ability to assess data reflective of patient status as appropriate to maternity/newborn care.
  • Interprets appropriate information needed to identify patient requirements relative to age specific needs and provides care needed as described in department policies, protocols and procedures.
  • Demonstrates/verbalizes knowledge of specific policies, protocols, and procedures related to victims of abuse (i.e. physical assault, rape/other sexual molestation, and abuse of child or spouse as appropriate to maternity/newborn care.
  • Floats to other areas in department, within scope of practice, as requested by charge nurse/manager/director.
  • Performs other duties as directed by unit leader.
  • Performs bedside shift report, and gives pertinent, accurate and comprehensive bedside report to oncoming RN assigned to patient per hospital/unit process/policy.
  • Recognizes life threatening situations, performs CPR and assists as appropriate during crisis situations.
  • Provides prompt and effective follow-up to physician’s requests.
  • Administers medication by all routes within his/her scope of practice per physician order and documents appropriately as defined by hospital policy.
  • Assists with orientation of new employees.
  • Completes a competency skills checklist within 90 days of employment.  Maintains a current competency with nursing skills required for assigned unit/department and demonstrates knowledge regarding Standards of Care and Standards of Practice in assigned area.
  • Performs other duties as requested by unit leadership.

Qualifications

Education:

Graduate from an accredited school of nursing.

 

License/Cert:

Current license to practice as a Registered Nurse in the state of Nevada

Current Basic Life Support (BLS).

NRP required within three (3) months of Hire/Transfer

STABLE certification within six (6) months of hire/Transfer – One time only, not to be renewed.

 

Experience:

Minimum of 1-year recent transition nursing experience in comparable unit.
