PATIENT ADVOCATE / MAKE A DIFFERENCE Livanta is searching for empathetic, considerate, and responsive individuals to work directly with and assist Medicare beneficiaries. The ideal Patient Advocate has a background in the fields of Social Work or Human Services. The role of the Patient Advocate is to provide assistance to beneficiaries and families who have filed an appeal for their discharge or termination of services and to decrease the likelihood of readmission to acute inpatient care. Your objective will be to ensure that Medicare beneficiaries are discharged from a hospital after a proper medical review and to reduce readmissions by verifying that discharge instructions are fully explained and understood. Livanta LLC is a Beneficiary and Family Centered Care (BFCC) Quality Improvement Organization (QIO) that serves Medicare beneficiaries in 27 states and US territories. The BFCC QIO program closely assists Medicare beneficiaries and their families in times of need. Patient Advocate Responsibilities --- Patient Advocate duties include: Provides education, advocacy, resource access, and targeted support to prepare the patient should their appeal be denied Promotes patient wellness by offering crisis intervention and emotional support Administers information regarding rights and responsibilities as patients covered by the Medicare program Patient Advocate Requirements: Bachelor---s degree or Master---s degree in social work or related field (e.g., Human Services) from an accredited college or university Current licensure in the State of Nevada preferred for those with a Social Work background Must have strong computer skills including the ability to type and speak on the phone simultaneously 2-4 years of experience in acute care case management, transition of care, social services Ability to work closely with all types of individuals Ability to be caring and empathetic toward others Livanta LLC---s Nevada offices are located centrally in Las Vegas --- easily accessible from all points.
