Job Description:
Case Management Supervisor I Annual Salary $43,259 A Qualified
Intellectual Disability Professional (QIDP) is responsible for
implementing strategies which: support the agency's mission; follow
agency policies and procedures; comply with Home and
Community-Based Waiver Services regulations; and align with
Person-Centered values and principles of self-determination.
Functions: Supervise and coordinate the implementation of supports
to all individuals receiving services on their caseload by:
Attending the PCP; Developing objectives and plans that have been
recommended in the Person Centered Plan (PCP); Ensuring service
design and delivery of the various supports, maximizing positive
outcomes.At least quarterly, observe and assess responses to
services and supports of the individuals on their caseload to
include: Satisfaction with services and quality of life; Review of
data and progress; Alerting team members to the changing needs of
an individual; Revising supports in accordance with those changes
upon approval of the PCP Team.Ensure staff receives required
training related to: The person-centered planning process; Rights
and due process; Positive behavioral interventions; The process for
development of outcome-based support plans.As Planning Documents
are developed or updated, monitor and ensure staff are alerted to
and trained on all new and revised support plans for each
individual on their caseload including the implementation of
required documentation and data collection.At least quarterly,
ensure all necessary current information and documentation is
present in the home files for staff review.Ensure consistency with
implementation of supports to individuals across all settings
(i.e., JDT, residential, recreational, medical, etc.), through
monitoring activities, reviewing case notes, and facilitating
communication between team members when necessary._ Qualifications:
_ Bachelor's Degree in related field. Two to four years of
experience in human services Minimum of one year of experience as a
Case Manager. Current CPR/First Aid Certification as required by
state/program. Self-motivated and detail-oriented with ability to
multi-task. Strong analytical and communication skills with ability
to build rapport with a team. Ability to exercise sound judgment
