Every position in our operation is critical and vital to the success of our operation and the well- being of those we serve.
We are expanding our staff to better serve our growing needs and we're looking for three new team members who: Are organized, well-disciplined, and able to manage multiple projects Have strong communication skills Display a warm and friendly attitude to everyone Have excellent reading, writing, and computer skills Have and follow a well-developed value system, are loyal, and interested in serving humanity Can quickly learn and use various computer programs and applications Are familiar with medical terminology and/or have recent experience in the medical field Understand and have been involved with maintaining confidentiality and compliance enforcement Our Care Team Specialists are involved in various daily activities including: Working on new patient admission packages and recertification of current patients Helping and preparing plans of care according to Dr. s orders Daily Patient Case Management documentation and record-keeping Periodic home visits to better serve our patients Accompanying patients to doctor appointments to advocate and explain program requirements Weekly conversations with patients and caregivers Requesting and researching medical records Job Requirements: This position requires the ability to work efficiently in a fast-paced environment while maintaining warm relations and dealing with multiple levels of health care workers.
Education and Training Requirements: Associate degree in a relevant field OR have relevant job experience and knowledge base.
Have a solid knowledge of professional writing and excellent use of computer programs and applications.
