hospital in the Las Vegas area is seeking a licensed Social Worker
to support patients and families through assessment, care
coordination, and safe discharge planning. This role focuses on
helping patients navigate complex medical and social situations
while ensuring appropriate resources and support are in place
throughout their hospital stay. The Social Worker plays an
important role in addressing psychosocial needs, providing
emotional support, and collaborating with clinical teams to help
ensure safe, comprehensive transitions of care. What You’ll Do
Assess patient and family needs related to illness,
hospitalization, and discharge planning Coordinate safe and
appropriate discharge plans using hospital and community resources
Provide emotional support and counseling to patients, families, and
significant others Collaborate with physicians, nurses, case
managers, and interdisciplinary care teams Assist patients with
access to social services, benefits, and community-based programs
Document assessments, plans, and interventions clearly and
accurately Support patients during crisis situations using
appropriate intervention techniques Maintain compliance with
hospital policies, patient rights, and regulatory requirements What
We’re Looking For Bachelor’s degree in Social Work or equivalent
education At least one year of professional social work experience
in a hospital or social service agency Relevant internships or
field work may be considered Active Nevada Social Worker license
Additional Experience That’s Preferred Three years of acute care
experience as a Social Worker Strong familiarity with
hospital-based discharge planning and care coordination Training
Underfill Consideration Candidates with recent, documented
practicum experience under a hospital Social Services department
may be considered for training Skills and Knowledge That Matter
Solid understanding of social work principles, healthcare systems,
and patient advocacy Knowledge of federal, state, and local social
service programs and community resources Familiarity with medical
terminology, diagnoses, and treatment processes Strong assessment,
interviewing, and counseling skills Ability to support patients and
families during high-stress or crisis situations Clear written and
verbal communication skills across diverse populations Ability to
build effective working relationships with clinical and
non-clinical teams Comfort managing documentation, reports, and
care coordination tasks in a hospital setting
